Connecting Organizations and People to Empower Diabetes Prevention and Treatment (Connections)

The purpose of this NOFO is to support evaluation of the effectiveness of existing Community-Clinical Linkage or Social Connectedness programs to reduce disparities in diabetes risk factors, incidence, or complications.

Additionally, research supported by this NOFO will assess how various mechanisms


(e.g., improving specific social or environmental conditions, mental health, or self-efficacy) will impact diabetes outcomes.

Specifically, the results of this funded research NOFO will:
(a) guide future scaling regarding programs using Community-Clinical Linkage and Social Connectedness; (b) demonstrate mitigation of observed disparities in diabetes risk factors, incidence, and complications through Community-Clinical Linkage and Social Connectedness programs; and (c) inform the understanding of the mechanisms by which Community-Clinical Linkage and Social Connectedness programs achieve successful health outcomes and the features that characterize effective programs.
