The High Obesity Program (HOP).
Poor nutrition and low levels of physical activity affect overall health and are significant risk factors for obesity and other chronic diseases.

States affects 7 8. 6 million (35%) adults and 1 2. 7 million (17%) children and accounts for approximately $147 billion in annual health care costs.
HOP works at the county level to implement evidence-based strategies and leverage resources from stakeholders and sectors (e.g., agriculture, transportation, education, business, commerce, and housing) in their populations of focus related to poor nutrition and physical inactivity.
This five-year program will fund land grant universities to leverage community extension services to implement evidence-based strategies that improve nutrition, safe and accessible physical activity, including family health weight and early care and education programs in counties with an adult obesity rate of over 40%.
It will carry out activities that can inform future state and local level programmatic efforts and provides continued support to achieve optimal health for all Americans.
