Pacific Islands Supplemental Funding for Five-Year US Affiliated Pacific Island Collaborative Performance Agreement for Tobacco Control, Diabetes Prevention and Control, and the Behavioral Risk Factor Surveillance System

The purpose of the program is to create healthier communities through sustainable and innovative population-based approaches such as broad-based policy, systems, and environmental changes.

The U. S. Territories and jurisdictions funded under this FOA will work collaboratively to promote and sustain

credit:


policy change efforts of sufficient intensity and duration to help achieve the “Healthy People 2010” objectives related to nutrition, physical activity, and tobacco control.

It is recommended that the U. S. Territories focus the interventions on populations who suffer disproportionately from the burden of disease.
