The purpose of this initiative for Sickle Cell Disease Treatment Demonstration Program is to develop and establish systemic mechanisms and partnerships to enhance access to services, prevention and treatment of sickle cell disease through coordination of service delivery, genetic counseling and testing,

bundling of technical services, training of health professionals, and efforts related to the expansion and coordination of education, treatment, and continuity of care for individuals of all ages with sickle cell disease.
The funded cooperative agreements will support the provision of coordinated, comprehensive, culturally competent and family-centered care for individuals of all ages with sickle cell disease through partnership networks comprised of Federally Qualified Health Centers (FQHC), non-profit hospitals or clinics and/or university health centers that provide primary care, working collaboratively with comprehensive sickle cell centers and community-based sickle cell disease support organizations or non-profit entities that work with individuals with sickle cell disease within the defined area.
The funded cooperative agreements will be involved in on-going data collection efforts as an evaluative method of the demonstration program
