This announcement solicits applications for the Sickle Cell Treatment Demonstration Program Regional Collaborative. The purpose of this funding opportunity is to:
(1) increase the number of providers that are treating sickle cell patients in the region; (2) increase the number of providers prescribing
disease-modifying therapies, such as hydroxyrea; and (3) increase the number of sickle cell patients that are receiving care from providers with greater knowledge of how to treat sickle cell disease.
This program will use collective impact strategies (see Collective Impact below) and collaborative learning to create Sickle Cell Regional Collaboratives that develop regional and sstate infrastructure to increase the number of providers providing disease-modifying therapies and enhancing the prevention and treatment of sickle cell disease through coordination of service delivery, genetic counseling and testing, bundling of technical services, training of health professionals, and efforts related to the expansion and coordination of education, treatment, and continuity of care for individuals with sickle cell disease.
